PCP/Medical Director Orientation To PACE

Our team of experienced medical consultants deliver timely, comprehensive education for providers new to your program or new to the PACE care model. The orientation provides the tools and information you need to succeed in PACE.

PCP / Medical Director Orientation to PACE
September 2024

  • Session One (two hours)
  • Thursday, Sep 5 | 2:30-4:30 p.m. (EDT) Introduction to the PACE Way
    • Introduce the history, philosophy, and basic structure of the PACE model of care, emphasizing the importance of the inter-disciplinary team and the provider’s role within this team.

Subsequent sessions meet for one hour, 3:30-4:30 p.m. (ET)

  • Tuesday, September 10 – Assessments and Care Plans
    • Discuss the governing regulations and components of an initial and six-month evaluation, the ways that assessments relate to IDT care planning, and the provider’s role in the IDT care planning process.
  • Thursday, September 12 – Advance Care Planning
    • Review the process for advance care planning discussions including prognostication, decisional capacity, and what makes for a successful family meeting.
  • Tuesday, September 17 – PACE Regulations, Service Delivery Requests
    • Review pertinent CMS regulations including the participant bill of rights, the process for service delivery requests, appeals, and grievances, and the role of a PACE quality improvement team.
  • Thursday, September 19 – Utilization Management in PACE
    • Discuss successful pro-active strategies for optimizing medication management and external referrals, and avoidance of unnecessary ED visits and hospitalizations.
  • Tuesday, September 24 – Geriatric and Palliative Care Principles
    • Discuss the concept of frailty and its association with other geriatric syndromes, basic palliative care principles, and the right to self-determination.
  • Thursday, September 26 – The PCP’s Role in Revenue and Expenses; Your PACE Day
    • Discuss the basics of risk adjustment and how PACE programs bring in revenue, as well as strategies to improve your day-to-day efficiency in the PACE clinic.
