Board-certified pharmacists

An assigned team of CareKinesis pharmacists and technicians collaborates with the clinical team to help optimize medication-related outcomes and streamline pharmacy care. Our PharmDs are board certified in geriatric pharmacy and conduct one-on-one consultations for personalized therapy recommendations.

Around-the-clock support

CareKinesis has a dedicated after-hours team. Our on-call service connects clinical teams with our own team of PharmDs for 24/7 medication management support in real time to ensure access, identify potential issues in complex therapies, and reduce risks.

Medication safety reviews

Medications are the primary intervention and represent the highest risk in caring for PACE participants. Our comprehensive MedWise Safety ReviewsTM help to assess medication risks to enable safer, more effective drug medication regimens that reduce unnecessary medical expenditures. Medication reconciliation

CareKinesis incorporates an all-encompassing plan of care for participants upon enrollment in PACE or other value-based health plans. We offer real-time medication reconciliation for participants, reducing the risk of errors, improving outcomes, and saving time. Additionally, our on-site orientation and transition services ensure smooth transitions of care, with personalized programs and education for PACE participants, providers, and administrators. For inpatient care, medication reconciliation and reviews are performed as close to hospital discharge as possible to identify at-risk medication regiments and reduce the likelihood of readmissions.1 Each of our services is driven by our EireneRx technology that integrates our electronic health records (EHRs).

Adherence packaging

What makes our adherence packaging different than others? What if participants have difficulty managing their medications at home? What if adherence packaging is not enough? MedaCube, a CareKinesis preferred partner, offers an in-home dispensing system that supports independent living. This 10-inch, cube-shaped device provides real-time notifications to caregivers or nurses when a patient misses a medication dose.

Reporting

CareKinesis collaborates with pharmacy benefit managers to reduce the number of vendor interactions and administrative costs associated with Medicare Part D. We provide robust clinical data on medication administration records, discontinued medications, controlled substances, drug classes, and more.

With EireneRx, CareKinesis evaluates risks for medication-related problems, including falls, using targeted medication reviews. Results of these reviews are used to modify drug regimens to mitigate risk for falls, hospital admissions, emergency department (ED) visits, and premature death. These improved outcomes also reduce medical expenditures.
