Winter 2025 Course Schedule
PCP / Medical Director Orientation to PACE
January 2025
- Session One (two hours)
- Thursday, January 9 | 2:30-4:30 p.m. (EDT) The PACE Way and Your PACE Day
- Introduce the mission and basic structure of the PACE model of care, emphasizing the provider’s role within the inter-disciplinary team. Concepts of participant dignity of risk and safety. Strategies for your success as a provider in PACE.
Subsequent sessions meet for one hour, 3:30-4:30 p.m. (ET)
- Tuesday, January 14 – PACE Revenue and Expenses
- Discuss the basics of risk adjustment and how PACE programs bring in revenue, as well as concepts and strategies that lead to more strategic and effective utilization of services.
- Thursday, January 16 – Assessments and IDT Care Plans
- Discuss the components of an initial and six-month assessment, how your assessments relate to the IDT care plan, and the provider’s role in the IDT care planning process.
- Tuesday, January 21 – Geriatric and Palliative Care Principles
- Discuss the concept of frailty and its association with other geriatric syndromes, basic palliative care principles, and medication de-escalation.
- Thursday, January 23 – Advance Care Planning
- Review the process for advance care planning discussions including prognostication, decisional capacity determination, and what makes for a successful family meeting.
- Tuesday, January 28 – PACE Regulations
- Review pertinent CMS regulations including the participant bill of rights, the process for service determination requests, appeals, and grievances, and the role of a PACE quality improvement team.
- Thursday, January 30 – Two sessions will run concurrently (choose one):
- Review and Final Jeopardy for PCPs
- Review session, delivered via a fun, interactive, game-style presentation
- Quality and Utilization Review for Medical Directors
- Discuss the medical director’s role as the leader of the QI team, as well as the importance of a structured system for evaluating and responding to utilization data.
